Rhinoplasty, including nose augmentation and nose reshaping surgery, has become a recent trend. For those who wish to do nose augmentation, it is recommended that they consult with surgeon to evaluate eligibility and outcome. For some who wish to have a very high nose, they should ask surgeon to help determine whether the desired nose shape goes with their face shape, as there is a chance that the nose can appear unnatural if the forehead is flat. In some cases, the patient might also be recommended to do nose wings surgery in addition to nose augmentation for the best aesthetic appearance. Apart from those mentioned, thickness of nasal skin is also considered an important factor in rhinoplasty surgery. For patients with thin nasal skin, there is a chance that the implanted silicone might extrude. For all these reasons, the patient should consult with doctor before deciding to do rhinoplasty surgery.      

There are 2 techniques in rhinoplasty as follows:

1. Closed Technique (surgical incisions positioned inside the nostrils): This surgery technique is popular in Thailand. The nose is augmented by insertion of silicone through the nostrils, from the nose bridge to the nose tip. With this technique, the implanted silicone should not be too long or there is a chance of extrusion.

2. Open Technique (surgical incisions positioned both inside and outside): This technique is for those who require reshaping of nose tip, both for shorter and longer nose tip. This technique might leave surgical scar on the nose base.

Preparation for rhinoplasty

First of all, the patient is recommended to discuss the desired nose shape with the surgeon. The surgeon will evaluate the condition and inform patient about the limitations in nose augmentation, as well as complications that might occur after the surgery. The surgeon will also give recommendations about rhinoplasty.
1. Before surgery, it is recommended to ensure that you have had a meal and are well hydrated

2. Patient should inform the doctor about any congenital disease

3. Avoid supplements and coagulent drugs
4. Abstain from alcohol and smoking 1 week before the surgery 

Procedure in rhinoplasty

1. Surgeon will evaluate shape of the nose and compare with face shape
2. Before the surgery, the surgeon will clean the nose and surgical area. The patient is recommonded to lay still
3. The surgeon will inject anesthetic around the nose area. In some cases, sleeping drug will also be injected for relaxation of the patient

4. After injecting anesthetic, the surgeon will make incision at the nose from nostrils, opening up the tissue and inserting prepared silicone (The surgeon will carve the silicone while waiting for the effects of anesthetic) 

5. After the silicone is inserted, the surgeon will check the position of the silicone. In case the silicone does not perfectly fit to the nose shape, the surgeon might remove the silicone and reshape it or reshape it while inside the nose until perfectly positioned and proceed to suture. 

Treatment after rhinoplasty

The recommendations after rhinoplasty are as follows: 
1. Patient should sleep with their head elevated, 2 pillows might be used
2. During 1 week after the surgery, patient should sleep on their back 
3. Cold pack should be used around nasal area during 1-2 days after the surgery to reduce inflammation and swelling

4. Patient is recommended to wash their face to prevent bacteria infection. The nostrils should be cleaned with mild soap
5. It is recommended to avoid knocking when doing sport or playing with pet 
6. Avoid rubbing the nose. In case there is blood or lymph, patient should gently clean it with cloth or tissue paper

7. Do not blow your nose. Instead, take a decongestant medicine

8. During the period of 2 months (8weeks) after the surgery, patient should avoid being under sunlight for too long 
9. Avoid strenuous physical exercises such as swimming or running for about 1 month after the surgery. Patient should not put their face down for too long  
10. For those who wear eyeglasses, it is recommended to use contact lens during 4-6 weeks after the surgery to avoid nose distortion. If necessary, patient should fix the glasses on their forehead to help reduce pressure
11. Refrain from alcohol as well as spicy and fermented food in order to avoid infection 
12. Patient should meet the surgeon regularly after the surgery for follow up and evaluation

Is revision rhinoplasty possible?

In the event that the outcome does not meet satisfaction, the patient can choose to do a revision. It is recommend to wait for 6-8 months for a more accurate assesment, as the nose requires time to heal. In case of nose tip reshaping that involves cartilage tissues, the revision might not be possible as the implanted cartilage will merge to nasal tissue

Possible complications after the surgery

There is a chance that complications such as nose distortion, infection, or silicone extrusion might occur after nose augmentation. The following are recommendations in case complications occur:
1. Distortion of nose – this might occur if the silicone implanted was too long, was inappropriately carved or the nose was accidentally hit after the surgery. In this case, surgeon will try to adjust the silicone back to its position. Revision is recommended for the case of nose distortion that occurs much later after the surgery
2. Infection after surgery – if patient experiences pain, it is recommended to check whether the nose is red or swollen. In case of swollen nose, the silicone needs to be removed and patient should wait until recovered before the revision. Infected nose should not be left untreated, as extrusion might occur and it might prolong the healing process
3. Silicone extrusion – the body treats implanted silicone as a foreign object, and thus there is a chance that resistance might occur and if the silicone implanted is too large, there is a risk of extrusion. It is recommended to choose appropriate size of silicone
